FCA — Failure Cause Analysis

A structured investigation to determine the causes of equipment or system failures.

Definition

Failure Cause Analysis (FCA) is the systematic examination of equipment failures to identify the immediate, contributing and underlying causes that resulted in the failure event. FCA uses engineering evidence, operating history, inspection findings and technical analysis to determine why failures occurred and to recommend corrective actions that reduce recurrence.

Why It Matters

Failure Cause Analysis supports reliability improvement by identifying failure mechanisms and enabling corrective actions that improve equipment performance and reduce future failures.

In Practice

FCA investigations commonly include inspection results, operating history, maintenance records, material analysis and engineering evaluation to determine the sequence of events leading to failure.
